الصداع النصفي (الشقيقه) هو السبب الاكثر شيوعا للصداع، تقريبا 50 % من المصابين بالشقيقه يتم تشخيصهم خطأ على أنهم يعانون من صداع التوتر، الصداع اليومي البسيط أو الصداع المتعلق بالاجهاد.

ثلاثة اسئله بسيطة يمكن ان تساعد في تحديد ما اذا كان صداعك ناتج من الشقيقه أم لا:

1. هل منعك الصداع من ممارسه انشطتك ليوم كامل أو أكثر في آخر ثلاثة أشهر؟
2. هل تشعر بالغثيان عندما يكون لديك صداع؟
3. هل الضوء يزعجك عندما يكون لديك صداع؟

إذا كانت الاجابه ( نعم ) لهذه الاسئله الثلاثة فإن احتمال وجود الشقيقه عندك يصل الى 90 %. التشخيص السليم هو الخطوة الاولى لوقف المعاناة غير الضروريه

العلاج:

اختيار استراتيجية لعلاج امراض الشقيقه يعتمد على شده الصداع و مدى تكرار حدوثه وغيرها من الظروف الطبية الخاصة بك. مجرد وصف الادوية المضاده للألم لعلاج الصداع أكثر من مرتين أو ثلاث مرات في الاسبوع أو في كميات زائدة قد تتسبب فى حدوث مضاعفات خطيرة تشمل زياده الصداع نفسه فيما يعرف بالصداع المرتد Rebound headaches

الادوية :

هناك مجموعة متنوعة من الأدوية قد صممت خصيصا لعلاج امراض الشقيقه. بالإضافة إلى ذلك فهناك بعض الادوية الأخرى التي قد تساعد أيضا في تخفيف أو منع نوبات المرض. الادوية المستخدمة لعلاج الشقيقه تندرج في فئتين رئيسيتين :

1. ادويه وقائية :

هذه الانواع من الادوية تؤخذ بانتظام ، في كثير من الاحيان على أساس يومي ، للحد من شدة أو تواتر امراض الشقيقه. من أشهر الأدوية المستخدمة لذلك مضادات المستقبلات بيتا (Beta blockers)، مضادات قنوات الكالسيوم (Calcium channel blockers)، مضادات الاكتئاب (Tricyclic antidepressants)، أدوية الصرع (Anti-Seizure) و السيبرو هيبتادين (Cyproheptadine) أحيانا في الأطفال. بالاضافة الى ذلك فإن الجرعات العالية من بعض الفيتامينات والمعادن قد تفيد في بعض الافراد. 2. ادويه لعلاج نوبه الشقيقه :

للحصول على افضل النتائج يجب أن تأخذ هذه الأدوية في اسرع وقت اذا واجهتك علامات او أعراض الشقيقه. وقد يكون من المفيد الراحة أو النوم في غرفة مظلمة بعد أخذها. من أشهر الادوية الموصوفه لعلاج نوبه الشقيقه التريبتان (Triptans). بالنسبة لكثير من الناس الذين يعانون من الشقيقه Triptans هي الدواء المفضل. هناك أيضا بعض الادوية الأخرى مثل العقاقير المضاده للالتهابات (Nonsteroidal)، الادوية المضاده للغثيان وغيرها من ادويه الألم.

Migraine Headaches are the most common cause of headaches. Migraines are misdiagnosed in about 50 % of patients as having as tension headache, simple daily headache or stress related headaches.

Three simple questions can help determine if your headaches are actually migraines :

1. Has a headache limited your activities for a day or more in last three months?
2. Are you nauseated or sick to your stomach when you have a headache?
3. Does light bother you when you have a headache?

Treatment

Treating migraines has to be approached in a specific way, dealing with migraines as just headaches and prescribing headache or over-the-counter pain medications more than two or three times a week or in excessive amounts, may cause a serious complication known as rebound headaches. Rebound headaches occur when medications not only stop relieving pain, but actually begin to cause headaches. You then use more pain medication, which traps you in a vicious cycle.

Choosing a strategy to manage your migraines depends on the frequency and severity of your headaches, the degree of disability your headaches cause, and your other medical conditions. You may be a candidate for preventive therapy if you have two or more debilitating attacks a month or if you use pain-relieving medications more than twice a week.

Medications:

A variety of medications have been specifically designed to treat migraines. In addition, some drugs commonly used to treat other conditions also may help relieve or prevent migraines. Medications used to combat migraines fall into two broad categories:

1- Preventive medications:

These types of drugs are taken regularly, often on a daily basis, to reduce the severity or frequency of migraines. Commonly prescribed preventive medications: Beta blockers, Calcium channel blockers, Tricyclic antidepressants, Anti-Seizure medications, Cyproheptadine (sometimes in children), Botulinum toxin type A (Botox). 2- Migraine-relieving medications (acute treatment) :

For best results, take pain-relieving drugs as soon as you experience signs or symptoms of a migraine. It may help if you rest or sleep in a dark room after taking them. One of the most Commonly prescribed migraine medications is Triptans. For many people with severe migraine attacks, Triptans are the drug of choice. Other medications include Nonsteroidal anti-inflammatory drugs (NSAIDs), Ergots. Anti-nausea medications, Butalbital combinations, and other pain medications.
